    Initial Certification and Renewal Applications

    To obtain copies of your Renewal Application or Initial Certification application, you must contact DEP Operator Certification staff at 717-787-5236 or email them here. Provide your name, client ID, and what type of application you need.

    The only types of certification applications you can download from this webpage are the upgrade/downgrade and the reciprocity applications.
