    The Office of Children, Youth and Families (OCYF) presents training on completion of Appendix A and Appendix B of Bulletin 3170-09-01 Out-of-Home Placement Services. These training PowerPoint videos offer in-depth instruction on completion of the attached forms for agencies who offer Out-of-Home Placement services. The instructors who present the training are staff members who wrote the Bulletin 3170-09-01 and can be consulted for further technical assistance. The contact information to obtain further assistance in completing these forms is included in the presentation.
