    Governor's Requisition Petition Samples

    There are five basic types of a petitions for a Governor's Requisition. 

     The links below will provide a copy of the standard end product for the various types of petitions in MS Word format.  

     

    The accused has open charges.

    Download Type 1 Sample

    The accused plead guilty to charges and failed to appear for sentencing.

    Download Type 2 Sample

    The accused was not present in the Commonwealth of Pennsylvania at the time of commission of the crime.

    Download Type 3 Sample

    The accused was found guilty in absentia and is needed for sentencing.

    Download Type 4 Sample

    The accused was sentenced and violated probation and/or parole.
