You can file a complaint against a healthcare facility with the state for many reasons. These include:

You can file a complaint against a healthcare facility by filing out the online complaint intake form. You can make a new complaint, or follow up on a recently lodged complaint. You can also request that your complaint is confidential. The DOH will still get in touch with you regarding the investigation, but the facility you make the complaint against will not.
