The Bureau is required by law to investigate all fatal and serious accidents that occur at underground Commonwealth mines. Investigation of these accidents requires the time and effort of a team of Mine Safety personnel. A report is made outlining the cause of the accident and possible means of preventing a similar accident. Incidents where there are no injuries such as ignitions and roof fall accidents also require investigation and reporting. These investigations can lead to legal action such as revocation of plan approvals, equipment approvals, mine official certificates, and other civil and criminal remedies.
