A permit application package must contain all applicable modules as well as the rest of the requirements found on the PWS Permit Applications webpage.

The individual modules may be found in the DEP eLibrary as Public Water Supply Application Modules 1-to-15 (3900-PM-BSDW0254B through 3900-PM-BSDW0254Q). It is only necessary to fill out the modules relative to the current project.

Module 1: Completeness Report (3900-PM-BSDW0254B)
Module 1 serves as a completeness checklist of required items that must be included in the permit application package. It is required for all application package submittals.

Module 2: Security and Operations (3900-PM-BSDW0254C)
Module 2 functions as a summary of risk and resilience evaluation, emergency response, security, and general planning and operations.

Module 3A: Groundwater Sources (3900-PM-BSDW0254D)
Module 3A is for proposed groundwater or groundwater under the direct influence of surface water (GUDI) sources. It provides a summary of the groundwater source, including water-quality characteristics, source-quantity characteristics, construction attributes, and general hydrogeologic characteristics. This module must be completed by a Pennsylvania-licensed professional geologist and accompanied by a Hydrogeologic Report.

Module 3B: Surface Water Sources (3900-PM-BSDW0254D)
Module 3B is for proposed surface water sources. It summarizes the water-quality characteristics, source-quantity characteristics, and construction attributes of the proposed surface water source.

Module 3C: Interconnection (3900-PM-BSDW0254D)
Module 3C is for proposed interconnection(s) between two public water systems. This module aids to summarize the interconnection details, including purchasing agreements, materials requirements, a quantity analysis, a Simultaneous Compliance Analysis, a Comprehensive Monitoring Plan, and an Inter-Water System Coordination Plan.

Module 4: Pretreatment (3900-PM-BSDW0254E)
Module 4 is intended for systems using pretreatment processes in the form of presedimentation, mixing and flocculation, chemical application, and sedimentation. Information in this module provides the design characteristics of the pretreatment facilities and must be accompanied by appropriate plans and specifications.

Module 5: Suspended Solids Contact Units (3900-PM-BSDW0254F)
Module 5 is intended for systems that use suspended solids contact units. This module summarizes information on contact units for flocculation and sedimentation prior to filtration and must be accompanied by engineering studies and appropriate plans and specifications.

Module 6: Softening (3900-PM-BSDW0254G)
Module 6 is intended for systems that use water softening in the form of lime-soda processes or cation-exchange softening. This module summarizes the proposed softening methods used and must be accompanied by appropriate plans and specifications.

Module 7: Filtration (3900-PM-BSDW0254H)
Module 7 is for systems using filtration in the form of rapid rate gravity, slow sand gravity, pressure filtration, diatomaceous earth, direct filtration, or membrane filtration. This module provides a summary of the proposed filtration technique and must be accompanied by appropriate plans and specifications.

Module 8: Disinfection (3900-PM-BSDW0254I)
Module 8 is intended for systems that use disinfection in the form of chlorine dioxide, liquid or gas chlorine, calcium or sodium hypochlorite, chloramines, ozone or UV disinfection. This module provides a summary of the proposed disinfection treatment technique and must be accompanied by appropriate plans and specifications.

Module 9A: Iron and Manganese (3900-PM-BSDW0254J)
Module 9A is intended for systems proposing iron and manganese removal in the form of aeration, chemical oxidation, lime-soda softening, manganese greensand filtration, ion exchange, polyphosphate sequestration, or sodium silicate sequestration. This module provides a summary of the proposed iron and manganese removal treatment technique and must be accompanied by appropriate plans and specifications.

Module 9B: Nitrate Removal (3900-PM-BSDW0254J)
Module 9B is intended for systems that need nitrate removal to keep concentrations below the primary maximum contaminant level. This module provides a summary of the proposed treatment technique and must be accompanied by appropriate plans and specifications.

Module 9C: PFAS Removal (3900-PM-BSDW0254J)
Module 9C is intended for systems proposing treatment to remove PFAS using GAC or ion exchange in treatment trains of at least 2 (two) pressure vessels in series. This module provides a summary of raw water quality and feed water quality if the system has existing pre-treatment prior to the proposed PFAS treatment vessels. Additionally, this module provides a summary of the proposed design for the selected PFAS treatment technique and must be accompanied with appropriate plans and specifications. Operating procedures for different vessel operating scenarios may be requested for additional evaluation.

Module 10: Corrosion Control (3900-PM-BSDW0254K)
Module 10 is intended for systems using corrosion control measures. This module provides a summary of corrosion control techniques and equipment and must be accompanied by appropriate plans and specifications.

Module 11: Organics Removal (3900-PM-BSDW0254L)
Module 11 is intended for systems that need organics removal to comply with a primary maximum contaminant level. This module provides a summary of methods and equipment for organics removal and must be accompanied by appropriate plans and specifications.

Module 12: Chemicals and Chemical Handling (3900-PM-BSDW0254M)
Module 12 is used to assess general information regarding chemicals. The module identifies the chemicals used and how they will be handled and introduced into the system.

Module 13: Pumping Stations (3900-PM-BSDW0254N)
Module 13 is used to assess information on pumping stations. This module serves to identify the location of pumping stations, details regarding the pumping station building(s), pumping units, power source(s), and information demonstrating appropriate design.

Module 14: Reservoirs, Elevated Tanks, Standpipes, and Pressure Tanks (3900-PM-BSDW0254O)
Module 14 covers new reservoirs, elevated tanks, standpipes, and pressure tanks and summarizes design information for water storage. This module should be submitted for new reservoirs, elevated tanks, standpipes, or pressure tanks or any proposed modifications to such units that could impact the quantity or quality of the drinking water.

Module 15A: Distribution Systems (3900-PM-BSDW0254P)
Module 15A covers design information for new distribution systems. This module assesses the distribution system design and construction to ensure it does not pose a threat to public health.

Module 15B: Bulk Water Loading Stations (3900-PM-BSDW0254Q)
Module 15B is intended for proposed bulk water loading stations. This module aids in assessing the reliability and sanitary, safety and security measures in place for mobile and fixed loading stations. This module must be signed by a Pennsylvania-licensed professional engineer.
